Quality Manager during round in wards follow these parameters

 

📝 1. Patient Care and Safety

✅ Patient identification bands (name, UHID, allergy band if applicable)
✅ Bedside care plan displayed and updated
✅ Proper positioning and comfort of patient
✅ Fall risk assessment and preventive measures
✅ Restraint use documentation if any
✅ Presence of emergency call bell within patient reach


📝 2. Documentation and Records

✅ Nursing notes updated timely
✅ Doctor’s progress notes with date and time
✅ Medication chart (prescriptions legible, signed, dated)
✅ Consent forms filled and signed wherever needed
✅ Intake-output chart maintained for relevant patients
✅ Vitals monitoring chart updated as per care plan


📝 3. Medication Management

✅ No expired medicines in ward stock
✅ Medicines labelled properly (name, dose, expiry)
✅ Narcotic drugs stored securely with proper register
✅ Correct storage of temperature-sensitive items in refrigerator with temp log
✅ No look-alike, sound-alike (LASA) errors in storage


📝 4. Infection Control Practices

✅ Hand hygiene compliance among staff
✅ PPE usage where required (masks, gloves, apron)
✅ Biomedical waste segregation at point of generation
✅ Sharp disposal practices followed
✅ Cleanliness of ward, beds, and surroundings
✅ Linen management (clean/soiled linen segregation)


📝 5. Equipment and Utility Check

✅ Emergency trolley (crash cart) checklist updated
✅ Oxygen cylinders with sufficient stock, labelled full/empty
✅ Suction machine functional
✅ Fire extinguisher expiry and accessibility
✅ Electrical plugs and medical equipment are safe and functional


📝 6. Patient Rights and Communication

✅ Patients aware of their treatment plan
✅ Food and diet provided as per dietician’s instructions
✅ Complaint/suggestion box visible and accessible


📝 7. Staff Awareness and Competency

✅ Staff knowledge on emergency codes
✅ Staff knowledge on infection control protocols
✅ Appropriate staffing as per duty roster
